Requirements for Provisional Registration

1. Copy of Primary Medical/Dental Qualification (e.g. MD, BDS, MBBS, MBChB.)
2. Copy of Registration and Licensing Certificates or evidence of registration from the country of origin or last country of practice.
3. Copy of a Certificate of Good Standing (sometimes called Certificate of Professional Status) from a Medical Regulatory Authority from the country of origin or last country of practice
4. Copy of passport information page.
5. Recent Passport size picture of not more than six (6) months old
6. Letter of intent from anticipated place of work
7. Proof of payment

Procedure for filling the online application form
