GUIDANCE FOR PROVISIONAL REGISTRATION APPLICATION PROCESS

The following instructions outline the procedure for applying for Provisional Registration (Housemanship Training Programme) through the MDCG online portal. The objective of the online application portal is to fast track the application process and ensure accurate information is retained by Council.:

You are to make payment into the MDCG Standard Chartered Bank Account and upload the bank pay-in slip.

Requirements for Provisional Registration

  1. Copy of Primary Medical/Dental Qualification.
  2. Transcript of programme.
  3. Curriculum of programme.
  4. Copy of passport information page.
  5. A recent passport size photograph of not more than six (6) months.
  6. Proof of payment for Application Form and Administrative Cost.

Procedure for filling the online application form

1. Click the register now button below.
2. You are required to scan and upload copies of your qualifications, a recent passport size picture and proof of payment from bank(deposit slip)
3. Click “Review” once all required fields are complete.
4. Click “Submit” after confirming the accuracy of information.
